The Duplex® Procedure

Some skin concerns sit at more than one depth at once, with rough texture and sun damage on the surface and loose, thinning skin beneath it. Treating only one layer often leaves the other untouched. The Duplex® Procedure combines radiofrequency microneedling with fractional CO2 laser resurfacing in a single session, addressing deeper laxity and surface texture together. Patients visit the Beverly Hills office from Brentwood, West Hollywood, Santa Monica, West LA, and across Los Angeles for the Duplex® Procedure.

Dr. Kami Parsa developed the Duplex® Procedure and lectures on it under the title The Duplex® Procedure: A Combination Approach to Facial Rejuvenation. He has also lectured on Combination Treatments for Skin Rejuvenation, the broader principle behind pairing technologies that work at different depths. As an oculoplastic and reconstructive surgeon, he brings particular care to treatment around the eyes, where laser and radiofrequency energy must be used with precise settings and eye protection. He is an Adjunct Clinical Professor at the Keck School of Medicine of USC.

“I designed the Duplex® Procedure because patients kept needing two treatments and two recoveries. Treating the deep layer and the surface together gives the skin one healing period and a more complete result.” — Dr. Kami Parsa

How the Duplex® Procedure Works

The Duplex® Procedure combines two established technologies:

  • Radiofrequency microneedling: Fine needles deliver radiofrequency energy beneath the surface, heating the deeper skin layers to tighten tissue and stimulate collagen.
  • Fractional CO2 laser resurfacing: The laser removes microscopic columns of damaged skin at the surface and upper layers, prompting fresh skin to form and improving texture, fine lines, and sun damage.

Both devices are FDA-cleared, and the Duplex® Procedure is a treatment protocol that combines them in one session so the skin goes through a single recovery period. You can learn more about each technology on the Morpheus8 and CO2 Skin Resurfacing pages.

What the Duplex® Procedure Can Address

  • Mild to moderate facial skin laxity
  • Fine lines, wrinkles, and sun damage
  • Uneven tone and texture and enlarged pores
  • Crepey skin around the eyes
  • Early softening along the jawline

Some patients with persistent under-eye swelling, including malar edema after previous filler, are evaluated for the Duplex® Procedure as part of their care. Results for swelling vary, and when filler is the cause, treating the filler itself is often the first step.

Is the Duplex® Procedure Right for You?

The Duplex® Procedure is generally best suited to adults with lighter to medium skin tones (Fitzpatrick types I to IV) who have mild to moderate laxity and texture concerns and can plan about a week of recovery. Because CO2 lasers carry a higher risk of pigment changes in darker skin, patients with Fitzpatrick types V and VI are usually offered radiofrequency-based alternatives instead.

The procedure is not appropriate if you have an active skin infection, are pregnant, have a pacemaker or other implanted electronic device, or have used isotretinoin recently. A history of cold sores, raised scars, or pigment problems should be discussed beforehand, since preventive medication or adjusted settings may be recommended. Patients with significant sagging or puffy eyelid bags usually get better results from surgery.

What to Expect Before, During, and After Treatment

Your visit begins with a consultation with Dr. Parsa or licensed members of his clinical team to review your skin type, history, and goals. On treatment day, a topical numbing cream is applied for about 45 to 60 minutes, and additional comfort options may be offered. Protective eye shields are used whenever treatment is near the eyes.

Expect redness, swelling, and peeling for about five to seven days, and most patients plan to take that time off from work and social commitments. Careful wound care and strict sun avoidance are important during healing, and the practice’s Pre and Post Procedure Instructions explain each step. Improvement is often visible within a few weeks, and collagen remodeling continues for several months.

Risks and When to Seek Care

Common effects include redness, swelling, oozing, crusting, and peeling during the first week, with pinkness that can linger for several weeks. Less common risks include infection, cold sore outbreaks, darkening or lightening of the skin, prolonged redness, and scarring. Because the procedure can be performed near the eyes, eye protection is essential.

Contact the office promptly if you notice increasing pain, spreading redness, pus, fever, blisters that resemble cold sores, or any eye pain or change in vision after treatment. Seek emergency care for sudden vision loss.

Frequently Asked Questions About the Duplex® Procedure

Medically reviewed by Kambiz K. Parsa, M.D. on

Why is this better than CO2 or RF alone?

CO2 laser improves surface texture and fine lines, while radiofrequency microneedling works deeper to tighten. Combining them treats both layers in one session with a single recovery. For deeper tightening alone, Morpheus8 may be enough.

Can it help with filler-related swelling under the eyes?

Some patients with persistent swelling after filler are evaluated for it, but results vary. When filler is the cause, dissolving it is often the first step, and your options are explained on the Malar Edema page.

Go To Top

Is this safe for darker skin tones?

CO2 lasers are not recommended for Fitzpatrick types V–VI due to risk of pigmentation changes. However, Dr. Parsa offers RF-only or alternative resurfacing protocols that are safe and effective for patients with richly pigmented skin.

Go To Top

How often should I schedule the treatment?

Most patients return annually for maintenance. Some may benefit from a series of treatments depending on age, skin condition, or concerns like malar edema.

Go To Top

Is the procedure painful?

Discomfort is manageable and short-lived. Most patients report a warm, stinging sensation that resolves within hours. The use of numbing agents and optional nitrous oxide enhances comfort throughout.

Words cannot express how thankful and appreciative I am to have found you. I am deeply grateful for you, for your generosity and caring nature as pertaining to my medical condition. As you know, I was apprehensive about having further surgery, but you are and have been reassuring and confident in the type of procedure you would perform in order to fix my eyelid from the beginning. The fact that you are not pushy and gave me the time and space to make the decision on my own allowed me to prepare myself mentally and emotionally, that really made a difference. Besides from that, your continuous generosity made me confident that you were the right doctor for the task at hand. You will never know how appreciative I am of that. I would also like to let you know how pleased I am with the results of the surgery. I have not felt this comfortable in years. My parents are also very grateful since this was my 14th surgery. You have a true gift. Thank you again for your patience, time, concern, and overall care. I pray that God continues to give you strength to help people who truly need life-changing medical procedures.

Dr. Parsa is an amazing doctor. Warm and very caring. I had my eyes done a couple years ago and I couldn't be happier. Dr. Parsa likes to take the conservative approach which I greatly appreciate in a doctor when it comes to cosmetic surgery. I also go to Dr. Parsa for Botox and he always knows just the right amount. The staff in Dr. Parsa's office are all terrific!
