CO2 Fractional Facial and Eyelid Resurfacing

Deep wrinkles, sun damage, acne scars, and crepey skin around the eyes often sit beyond the reach of gentle treatments. Fractional CO2 laser resurfacing removes microscopic columns of damaged skin and prompts the skin to rebuild with new collagen, producing some of the most noticeable non-surgical improvement in texture and fine lines. It can be performed on the face and on the delicate eyelid skin, either alone or alongside eyelid surgery. Patients visit the Beverly Hills office from Brentwood, West Hollywood, Santa Monica, Century City, and across Los Angeles for CO2 resurfacing.

Dr. Kami Parsa has lectured on Management of Eyelid Retraction, a complication in which the lower eyelid is pulled down and away from the eye. Aggressive laser treatment of the lower lid is one possible contributor, which is why eyelid resurfacing calls for careful settings and an understanding of how the lid is supported. He also developed the Duplex® Procedure, which combines fractional CO2 laser with radiofrequency microneedling, and he completed his ophthalmology residency at USC’s Doheny Eye Institute.

“The eyelid is the thinnest skin on the body, and it sits right on top of the eye. CO2 can do beautiful things there, but only when the settings respect how fragile that skin and its support really are.” — Dr. Kami Parsa

What Is Fractional CO2 Laser Resurfacing?

A fractional CO2 laser delivers energy in a grid of tiny beams. Each beam removes a microscopic column of skin through the surface and into the upper dermis, while the skin between the columns stays intact to speed healing. As the skin repairs itself, it replaces damaged tissue with fresh skin and new collagen, which improves texture, fine lines, and sun damage and can tighten the skin modestly.

Fractional CO2 lasers are FDA-cleared for skin resurfacing and the treatment of wrinkles. Because the treatment is ablative, meaning it removes skin, it offers stronger results than gentler lasers but requires about a week of recovery.

What CO2 Resurfacing Can Treat

  • Fine Lines and Wrinkles, including around the eyes and mouth
  • Crepey lower and upper eyelid skin
  • Sun damage and uneven texture
  • Acne scars and some surgical scars
  • Mild skin laxity

CO2 resurfacing is often combined with lower eyelid surgery to address the skin while surgery addresses the fat and support underneath. For tightening along with resurfacing, the Duplex® Procedure pairs CO2 with radiofrequency microneedling.

Is CO2 Resurfacing Right for You?

CO2 resurfacing may be a good fit for adults who:

  • Have moderate wrinkles, sun damage, or scarring
  • Have crepey eyelid skin without significant eyelid bags or sagging
  • Can plan about a week of recovery and strict sun avoidance
  • Have lighter to medium skin tones

CO2 lasers carry a higher risk of pigment changes in darker skin tones, so patients with Fitzpatrick skin types V and VI are usually guided toward options such as Morpheus8. CO2 resurfacing is not appropriate if you have an active skin infection, have used isotretinoin recently, or are pregnant. A history of cold sores, raised scars, vitiligo, or radiation to the area should be discussed beforehand. Patients with lower eyelid laxity or a history of eyelid surgery need careful evaluation before eyelid resurfacing.

What to Expect During CO2 Resurfacing

Your visit begins with a consultation with Dr. Parsa or licensed members of his clinical team, who review your skin, eyelid support, and history. Preventive antiviral medication is often prescribed to lower the risk of a cold sore outbreak. On treatment day, numbing cream is applied, and when the eyelids are treated, protective shields are placed directly over the eyes. Additional comfort options may be offered.

Expect redness, swelling, oozing, and peeling for about five to seven days, and plan that time away from work and social commitments. Careful wound care is important, and the practice’s Pre and Post Procedure Instructions explain each step. Pinkness can last for several weeks and is usually covered easily with makeup once the skin has healed. Collagen remodeling continues for several months, and strict sun protection helps preserve the result.

Risks and When to Seek Care

Common effects include redness, swelling, oozing, crusting, and peeling during the first week, with pinkness that can persist for weeks. Less common risks include infection, cold sore outbreaks, acne flares, darkening or lightening of the skin, and scarring. Around the eyes, rare risks include the lower eyelid pulling down or away from the eye.

Contact the office promptly if you notice increasing pain, spreading redness, pus, fever, or blisters that resemble cold sores. Seek prompt care for any eye pain, discharge, or change in vision after eyelid treatment, and emergency care for sudden vision loss.

Words cannot express how thankful and appreciative I am to have found you. I am deeply grateful for you, for your generosity and caring nature as pertaining to my medical condition. As you know, I was apprehensive about having further surgery, but you are and have been reassuring and confident in the type of procedure you would perform in order to fix my eyelid from the beginning. The fact that you are not pushy and gave me the time and space to make the decision on my own allowed me to prepare myself mentally and emotionally, that really made a difference. Besides from that, your continuous generosity made me confident that you were the right doctor for the task at hand. You will never know how appreciative I am of that. I would also like to let you know how pleased I am with the results of the surgery. I have not felt this comfortable in years. My parents are also very grateful since this was my 14th surgery. You have a true gift. Thank you again for your patience, time, concern, and overall care. I pray that God continues to give you strength to help people who truly need life-changing medical procedures.

Dr. Parsa is an amazing doctor. Warm and very caring. I had my eyes done a couple years ago and I couldn't be happier. Dr. Parsa likes to take the conservative approach which I greatly appreciate in a doctor when it comes to cosmetic surgery. I also go to Dr. Parsa for Botox and he always knows just the right amount. The staff in Dr. Parsa's office are all terrific!
